POLICY OVERVIEW
AlignSD Wellness Center complies with all applicable federal and California civil-rights laws. We do not discriminate on the basis of race, color, national origin, age, disability, or sex, and we never exclude or treat people differently for these reasons.

AlignSD Wellness Center
A. Reasonable Accommodations
As a small, privately funded practice, we will make reasonable efforts to accommodate individual communication needs. When a request exceeds the resources we have on-site, we can help patients locate third-party services (e.g., professional interpreters or translators) at the patient’s expense.
Communication for Patients with Disabilities
We will explore practical options such as written instructions, typed notes, or referrals to qualified sign-language interpreters.
Language Assistance
Upon request, we can refer you to professional interpreter or document-translation services in your preferred language.
B. Grievance Procedure
If you need assistance—or believe we failed to provide a reasonable accommodation—you may file a grievance with our Doctor. You can do so in person, by mail, fax, or email; our team is available to help you complete the process.
C. Civil-Rights Complaints
You may also file a civil-rights complaint with the U.S. Department of Health & Human Services, Office for Civil Rights, through the online portal (https://ocrportal.hhs.gov/ocr/portal/lobby.jsf) or by contacting:
U.S. Department of Health & Human Services 200 Independence Avenue SW, Room 509F, HHH Building Washington, DC 20201 1-800-368-1019 (Voice) • 800-537-7697 (TDD)
